  • Medical Certificate (issued within 3 months) + lab results
  • NSO-authenticated Marriage Certificate (if married)

  • Transfer credentials / Honorable Dismissal
  • Certification of Grades
  • Certificate of Good Moral Character or Compliance (if previously sanctioned)
  • Two (2) recent 2”x2” colored photos
  • Police Clearance (photocopy)
  • NSO-authenticated Birth Certificate (1 copy)
  • Medical Certificate + lab results
  • NSO-authenticated Marriage Certificate (if married)

  • Official Transcript of Records
  • Certificate of Good Moral Character

  • Written permit from the Registrar of the home school

  • Notarized letter request
  • General Application Form (BI Form No. MCL-07-01)
  • Medical Certificate from Bureau of Quarantine or government medical institution
  • Passport bio-page (photocopy)
  • National Intelligence Coordinating Agency Clearance
  • Bureau of Immigration Clearance Certificate
  • Alien Certificate of Registration (for visa renewal)
  • School credentials

Note: All foreign student documents must be Red Ribbon authenticated and bear the Dry Seal of the Philippine Embassy in the applicant’s country of origin. Refer to the CHED Guidebook for Foreign Students for complete procedures.

  2. Transfer Credential (For Transferee);
  3. Photocopy of PSA-issued Birth Certificate;
  4. If married, copy of Philippine Statistics Authority (PSA) authenticated marriage certificate.
  5. Entrance Examination;
  6. Two (2) copies of recent 2″ x 2” colored photo;
  7. Study Permit (for employed);
  8. Affidavit of no employment during the period of enrollment at ULS Graduate School;
